- Format MM/DD/YYYY. This fields needs the numbers for 'month' and 'date' to have two digits. If the number has only one digit, you must add a zero (0) before it. It's important to follow this exact format, as any incorrect format will not be accepted.
- Date of Examination
- Select your date of examination from the dropdown menu.
- Type of Examination
- Select the specific type of examination you have taken from the dropdown menu.
- CSC Office
- Click the dropdown menu and select the appropriate Regional Office that has jurisdiction over the testing center where you took the examination.
- Check Box
- Check the box to confirm that you accept the CSC Privacy Policy.
- Submit
